Job Title: Dementia Outreach Specialist – Dane County, Wisconsin
This position plans and implements the agency’s core services and programs in Dane County, Wisconsin. The position is full-time and paid on an hourly basis. The candidate will report to the Executive Director.

Essential Job Duties:

75% Assist individuals touched by Alzheimer’s disease or another dementia—including those with a diagnosis, their families, professionals, and communities—by:
• Providing information, support, education, and resource options to persons with dementia, their families, professionals, and community members.
• Offering consultations around difficult issues and decisions, future care planning, and barriers to care for individuals with dementia and their families.
• Acting as a resource for local service providers.
• Establishing and facilitating caregiver/care partner support groups.
• Updating and conducting education programs for persons with the diagnosis, families and friends, professionals, and the community.
• Developing and conducting programs for persons with a diagnosis, including memory cafés and support groups.
• Attending health fairs and other public events.
• Assisting with publicity efforts, including writing articles and making public presentations.
• Recruiting volunteers for ADAW programming.
• Advocating for individuals, families, and communities impacted by dementia.
• Completing agency paperwork, including case notes and session documentation.
• Attending staff and program meetings as required.
• Performing other duties as assigned.

25% Provide community outreach around dementia by:
• Cultivating community partnerships to identify needs of people with dementia and their caregivers/care partners, support research, and present best practice models.
• Creating new services or enhance existing local resources to meet community needs.
• Working with community coalitions and networks, ADAW program staff, and other community partners to plan, coordinate and present dementia-specific activities and events.
• Fostering integration of medical and social service systems to encourage early diagnosis, facilitate cross-referrals between medical and social support systems, and enhance service delivery.
• Raising awareness and funds to support ADAW programs and services. Work with local planning committees, ADAW development staff, and other community partners to plan and conduct fundraising events. Encourage and facilitate community partner involvement in increasing awareness of ADAW programs and services and fundraising opportunities.
• Performing other duties as assigned.

Education:
• Bachelor’s degree in social work, psychology, education, or health-related field is required; master’s degree is preferred.

Experience:
• One (1) year of social services or health-related experience is required.
• Previous experience with MS Office (Word, Excel, Outlook, PowerPoint) software is required.

Knowledge, Skills, and Abilities:
• Knowledge of Alzheimer’s disease and other dementias and their impact on families.
• Excellent interpersonal and verbal communication skills, with particular ability to work with individuals with dementia and their families.
• Understanding of and skills around group process and dynamics.
• Ability to foster community collaborations.
• Knowledge of service and healthcare delivery systems a plus.
• Ability to work independently with minimal supervision and within a staff team.
• Ability to balance and prioritize multiple responsibilities and work within deadlines.
• Professional and welcoming phone presence is required.
• Residence in assigned service area preferred.
• Ability to travel regionally is required.
• Flexible schedule is required as some activities will be scheduled on evenings and weekends.

Licenses and Certifications:
• Possession and ability to maintain a valid driver’s license and use of personal insured vehicle is required.

Physical Requirements and Work Environment:
• Ability to lift 20 lbs. Some setting up and dismantling of tables and exhibits required.
